The Next Step

You have completed the online enrollment form, and the next step is on us to review and confirm your enrollment.

If you enrolled in a Wellcheck EZ membership plan… we will confirm your enrollment shortly.

If you enrolled in a Pediatric Consult or Pediatric Consult (Legacy FFS) membership plan… If you are an existing, active consult patient, we will confirm your enrollment shortly. If you are a new patient requesting consult services, we will reach out to you to discuss your needs and forward our preliminary consult questionnaire to you to complete. After reviewing your completed questionnaire, should we determine that we are a good fit for your needs, we will confirm your enrollment, reach out again to schedule your initial consultation and forward our comprehensive consult questionnaire to you to complete before the initial consultation. If we determine that we are not a good fit for your needs, we will reach out again to discuss other options.

If you enrolled in a Primary Pediatric Care membership plan… Our Primary Pediatric Care membership slots are filled. If you were able to select a provider for your enrollment, then—Yay!—you enrolled at the opportune moment when a membership slot was open. We will confirm your enrollment shortly. If you were not able to select a provider for your enrollment, then your enrollment has been wait listed. When a slot becomes available for you, we will reach out to you to determine if you would still like to confirm your membership. If you confirm within 24 hours, we will confirm your enrollment, otherwise, we will move on to the next wait listed enrollment.

Once Confirmed

When your enrollment has been confirmed, you will receive an email signaling confirmation of your enrollment. If you are a new patient to us, look for an additional email invitation(s) to setup your patient portal account for interacting with our care team.
